The mark consists of a yellow circle with a thin brown border that contains six partial concentric circles that are brown, red, pink, orange, yellow, and beige in color and enclose at the bottom around a butterfly that is orange with brown lines in its bottom half and pink with brown lines in its upper half, has a brown outline and daisies with beige petals and orange centers within the outline of its wings, and has a brown center through which its red body extends with brown antenna directly underneath a large daisy in the center of the yellow circle that has beige petals with brown interior lines and outlining and an orange center also outlined in brown. The wording "HAZEL BOUTIQUE" appears in brown in the top portion of the yellow circle, and the in the "A" in "HAZEL" and the "B" and "O" of in "BOUTIQUE" all contain beige daisies with orange centers.
