The mark consists of a Phoenix bird in the center of two concentric circles; the words "Phoenix Physical Therapy" are on the outside of the smaller circle and the inside of the larger circle with the tip of the bird's tail and the top of the wings going into the lettering. The bird's head is facing left and the wings are outstretched to give the imagery of the bird rising.
