Personal care assistance with activities of daily living for senior persons and persons with disabilities; providing in-home non-medical care for elderly persons, namely, personal care assistance of activities of daily living, such as bathing, grooming, personal mobility, meal preparation, laundry, light housekeeping, shopping and running errands, medication reminders, and dementia care; social services, namely, finding permanent homes and coordinating living arrangements for the elderly and people with disabilities by matching those persons with home providers; social services, namely, support services for people with intellectual and developmental disabilities, namely, personal care management, household management, and guardianship services; social services, namely, in-home support services for people with intellectual and developmental disabilities, namely, self-help and self-care, assistance and skill-building with household activities and chores, transportation, and community integration; community outreach and social service programs, namely, providing independent living assistance services for and on behalf of seniors and people with disabilities in the nature of skill building for self-sufficiency, independent living and community integration; case management services, namely, coordination of personal support and care, vocational and education issues, and social, and psychological services for the elderly and people with intellectual and developmental disabilities
