Providing case management services, namely, coordinating legal, medical, physical, social, personal care and psychological services for persons with pain and or complex illness, including cases where there is coexisting medical and behavioral health diagnosis. Emphasis is on creating a multimodal, multidisciplinary treatment team and guiding the team toward a multimodal, individualized and patient centered careplan.
