The mark consists of two leaves, each ovate shaped with a single lobe, smooth margins and petiole, on the left side of the mark; the upper leaf is darker than the lower leaf; the two leaves are on the left side of the mark and they are next to each other, with the upper leaf's lower left side overlapping the lower leaf's lower right side and the petioles joining together where the bottom of the two leaves meet; to the right of the leaves there are three lines of text; the top line contains the letters "VNH" in large capital letters; the middle line contains the words "VISITING NURSE and HOSPICE" in a much smaller font, with all the words in capital letters except for the word "and" which is lower case; the bottom line "VNH" contain the words "for VERMONT and NEW HAMPSHIRE" with the words "VERMONT" and "NEW HAMPSHIRE" in all capital letters and the words "for" and "and" in lower case letters.
