The Dallas-Fort Worth area is recognized formally as the Dallas-Fort Worth-Arlington, TX Metropolitan Statistical Area, by the United States Office of Management and Budget, and constitutes following counties in Texas: Collin, Dallas, Denton, Ellis, Hood, Hunt, Johnson, Kaufman, Parker, Rockwall, Somervell, Tarrant, and Wise.

This additional information requested in support of the applicant's concurrent registration: 1) The services and the geographic areas for which applicant seeks registration of the mark - orthodontic services, in the Texas counties of Collin, Dallas, Denton, Ellis, Hood, Hunt, Johnson, Kaufman, Parker, Rockwall, Somervell, Tarrant, and Wise. 2) The mode of use of the mark is for advertising, including stationery, signs, and literature. 3) There are no exceptions to the claim of exclusive use in the geographic area for which registration is sought. 4) Dr. Duane Grummons, who owns S/N 85027504, registered as 3890201, "SMILE 4 LIFE", has consented to the use of the mark.
