The mark consists of a yellow triangle outlined in a double black line, against a white background, superimposed over a rectangle with a black outline, also with a white background, with the word "FLOOR" appearing in black block typeface to the left of the triangle, and the word "SAFETY" appearing in black block typeface to the right of the triangle. Inside the triangle is the falling stick figure in black. Gray shading appears along the bottom border of the rectangle/triangle design.
